Assessment

Free in-home consultation

We meet with the individual and their family to understand their daily routines, goals, and the level of support needed. We review the IPP (Individual Program Plan) and coordinate with the Regional Center service coordinator.

Matching

Staff matching and care planning

We carefully match each client with trained support staff based on personality, language, schedule, and support needs. A personalized care plan is created before services begin.

Services begin

Support starts in your home

Our staff come directly to the client's own apartment or home. We assist with activities of daily living (ADLs) such as bathing and grooming, and instrumental ADLs such as meal prep, grocery shopping, budgeting, and community outings — all at the client's pace.

Ongoing

Continuous monitoring and family updates

Our supervisors conduct regular check-ins, participate in IPP meetings, and keep families informed. We adjust the support plan as the individual's needs grow or change over time.
